How to Use Patient ID Code

The Patient ID code can be used by healthcare organizations, localities, churches, and any other group wishing to collect screening data. You can use one code for all your screenings from an organization to group them all together.

Examples

Hospitals and Clinics

Use your facility name or initials, followed by a number that you create and ensure you keep it unique. For example, Hope Hospital could use the code Hope-12345 where the 12345 is the unique number. You should hand this to the patient and ask them to bring it back with them for follow up screenings so you can see changes in their history.

Churches

Church name? Event?

Run number?

Public Health Departments

Public health agencies might want to have members of the public complete the screening, so there would not be a way to assign each person a unique ID. Instead, choose an identifier for your agency and ask all members of the public to use that as the Patient ID Code. For example, Hope County Department of Public Health could ask everyone to use HopeDPH as their ID code.
